Welcome to Pathways Brain Injury Program
The Pathways Brain Injury Program is located at the Louisville East Post-Acute Skilled Nursing Facility. The Pathways unit is an inpatient rehabilitation program having a specialty contract with Kentucky Medicaid and is licensed for a skilled nursing setting. Currently, Pathways is the only skilled nursing home brain injury unit in the state of Kentucky. Additionally, Pathways is one of 6 adult skilled nursing brain injury units accredited by CARF International.

Physical Therapy
Therapy focused on movement, with a program built to improve your balance, strength, coordination, and mobility after a brain injury.
Therapeutic Recreation
Recreation planned around your abilities and interests, so you can build new leisure skills and get back to the activities you enjoyed.
Medical Services
Our Medical Director is a board-certified physiatrist. Pharmacy, lab, X-ray, and wound care services are provided under contract.
