Welcome to Pathways Brain Injury Program
The Pathways Brain Injury Program is located at the Louisville East Post-Acute Skilled Nursing Facility. The Pathways unit is an inpatient rehabilitation program having a specialty contract with Kentucky Medicaid and is licensed for a skilled nursing setting. Currently, Pathways is the only skilled nursing home brain injury unit in the state of Kentucky. Additionally, Pathways is one of 6 adult skilled nursing brain injury units accredited by CARF International. CA
