What insurances do you accept?

We are proud to accept private pay, Medicare, Medicaid, Workmans Comp, commercial policies, private pay, and more. No matter what type of insurance you have, our insurance experts will take time to help you understand your plan benefits.

Do you offer short-term rehabilitation?

Yes. At Pathways, your successful recovery and quick return home is our top priority. We provide a unique personalized experience as you recover from your brain injury. Physical, occupational, and speech therapy is conducted under the guidance of licensed experts and is available Monday through Friday.

What is your visitor policy?

At Pathways, we believe that family and friends play a critical role in our residents’ emotional and physical well-being. During the week, we recommend waiting until after therapy has been completed for friends and families to begin visiting, as this allows persons served to fully immerse themselves in their speech, physical, occupational, and recreational therapies.

When will the doctor see me?

Our Medicaid Director is on-site Mondays, Tuesdays, Thursdays, and Fridays to see all persons served by our program. If there is an emergent need for the physician, he is also available by phone.

How often will I have therapy?

At Pathways, all persons served are scheduled for three hours of therapy daily, Monday through Friday, including one hour with physical therapy, one hour with occupational therapy, and one hour with speech therapy. Recreational therapy is also provided Monday through Friday. On occasion, therapy may be provided on the weekend due to holidays during the week.

How long is a typical stay?

Each case is different and unique. Patient progress is reviewed weekly by our Interdisciplinary Team (IDT) of nurses, therapists, and dietitians. Our main goal is that you reach your highest level of independence. The social worker will schedule monthly care plan meetings to discuss therapy progress, barriers, resources, with the goal of assisting persons served in discharge back to the community. Length of stay will be determined upon your participation in therapy, your progress in therapy, and your individual rehabilitation goals.

Are activities available?

We offer daily activities for a wide variety of interests. Pathways has its own Recreational Therapist, who is a COTA by background, who will assist you in participating in activities that interest you. You will also have the opportunity to participate in facility-wide scheduled events.

What is your dining program like?

At Pathways we’re proud to offer a full-service dining program with diets designed by our full-time, in-house Registered Dietician.

Do I need to bring in my medications?

No, you do not need to provide medications unless specifically told to do so. Upon admission, we will order your medications using our off-site pharmacy then securely store them on-site to ensure safety.

What type of clothing items should I bring?

We encourage you to bring a pair of comfortable shoes and at least seven days worth of comfortable clothing including pajamas and undergarments.

Can I bring personal belongings?

Yes! We encourage you to bring personal items such as family photos, blankets, and decorative items. Anything that will help make your stay more comfortable.

Can my loved one receive mail?

Mail is delivered to residents personally daily except for weekends and holidays.

How do you protect patients, staff, and visitors from COVID-19 and other infectious diseases?

Our team is highly trained and experienced in caring for high-acuity patients and infectious diseases like COVID-19. Our dedicated infection prevention nurse helps ensure we follow current infection prevention guidelines and best practices. This includes educating all staff about the appropriate use of personal protective equipment (PPE) such as N95 masks, surgical masks, gloves, and gowns and when to use COVID-19 protocols required by the local Department of Public Health and/or the CDC.
